A parent signing up a child, or yourself. One form for each person training, each with their own PIN. The academy confirms you before your first class.

Only if you are coming from another academy. Leave these on White if you are starting out.

When does your membership run out?

The academy needs a date. If you are paying as you go, put the date you mean to look at it again.

4 to 8 numbers. This is how you get back in on a new phone, so pick one you will not forget. It opens this one record, so a second child needs a PIN of their own.
